Job Title: Band 6 Occupational Therapist.
Team: Home First - Discharge to Assess and Prevention of Admission Teams.
Location: Citywide.
Work Arrangement: Full time (37 hours). Permanent contract. Hybrid working.
Role Outline: We are seeking a dedicated and dynamic Band 6 Occupational Therapist to join our supportive and friendly team. In this role, you will play a pivotal part in the Discharge to Assess and Prevention of Admission teams, contributing to the facilitation of discharges, focusing on rehabilitation and ensuring intervention is provided at the right time and in the right place.
This is an exciting time to join the Edinburgh Health and Social Care Partnership as we continue to develop our Home First approach.
Key Responsibilities:
1. Conduct thorough assessments of patients in the community to support successful discharges from hospital and prevent unnecessary admissions.
2. Implement rehabilitation strategies to promote independence and positive outcomes.
3. Supervise junior staff members and students, fostering their growth and development.
4. Engage in extensive training opportunities to refine your clinical skills.
5. Take advantage of a structured supervision framework, attending regular 1:1 meetings.
6. Develop leadership skills and contribute to the team's success by being a part of quality improvement projects.
