This is an excellent opportunity for an enthusiastic and committed person to join the Community Respiratory Service in Newhaven as a Band 3 Community Respiratory Rehabilitation Assistant. You will be working as part of a dynamic multi-disciplinary team comprising of nurses, physios, occupational therapist, cognitive behavioural therapist, associate practitioners and community rehabilitation assistants. CRS support chronic respiratory patients in the community. We work to help patients independently manage their lung condition on a day to day basis and to prevent unnecessary hospital admissions. The successful applicant will be responsible for following care plans given by all members of this multidisciplinary team. The role consists of home visits, teaching and assessing skills of self-management. You will also assist in pulmonary rehabilitation provision and clinics. There is also an administrative element to the position. We are looking for an excellent team player who has outstanding communication skills, good time management and organisational skills. These are all essential requirements of this post. It is essential that applicants hold a full UK driving licence and have a car available for work, as the successful applicant will be required to undertake home visits independently.
