North HUB CMHTs based at Northcroft are looking to employ a qualified staff to support and manage caseload of 30 clients with various mental health and social needs. Although the clinical work will be delivered with full autonomy, the post holder will work with the multidisciplinary team attitude in mind to best support their clients with a professional advice from other disciplines in a team.
Apart from being responsible for their caseload they will be actively supporting our established and hardworking Duty Team when working with individuals who are in crisis or in need of urgent interventions. Additional responsibilities will include cover of the Depot Clinic and completion of initial assessments for service users referred to secondary care.
Main duties of the job
The post holder:
Will act as an independent practitioner within a defined community setting, while liaising with other professionals as necessary
Will be responsible for the assessment, planning, implementation and evaluation of nursing care within agreed parameters, and have a working knowledge of local, national policies and legislation, which govern current service provision and audit.
Will be responsible for ensuring effective communication pathways exist.
Will be responsible for ensuring the promotion of equality diversity and rights in accordance with good practice and legislation.
Will be responsible for identifying and contributing to the training needs necessary in providing quality evidence based care as appropriate.
