The purpose of this post is to work as part of a team which provides mental health services to service users with complex needs and to focus on their recovery goals and day to day support and needs.
The role includes working across organisational and Team boundaries and is coordinated through the CPA process. The role is to provide support and give time to an allocated number of service users with complex needs and to promote their Recovery and maintain them in their community environment. There is also a specific focus on supporting service users to have the skills and confidence to move from higher level support to more independent living environments.
The STR worker also assists the service users to work with the care co-ordinators to, plan, implement and evaluate their individual and person centred care plans.
To support implementation of care in a range of community settings, including residential and nursing homes, day services and general public environments.
Participate in the assessment and care planning of service users' mental and physical health, including risk assessment and early signs of relapse and recovery planning
Provide care in accordance with individual service users' care plans.
Assist service users to access community resources that are culturally appropriate.
Ensure the right information is available to enable service users to gain access to resources.
· Assist service users in maintaining their dignity, privacy and choice, treating them with respect at all times.
· Support Service users to participate in recreational therapeutic activities.
· Work as part of a team, exchanging appropriate information with service users, carers, and other team members on a regular basis.
· Attend and contribute to staff and team meetings as required.
· Report any safety or security concerns in order to maintain the safety of service users, staff and others.
· Work in a professional manner, respecting the boundaries of the relationship with service users, carers, and staff, and maintaining confidentiality of sensitive information.
· Take part in regular supervision and appraisal
· Attend and participate in training courses and events as directed.
· Undertake any other reasonable duties as directed.
