We are looking for a creative, innovative and motivated practitioner to join our Symptom Management service. We are looking for someone who has clinical experience of working with a range of client groups, across all age pathways. Experience of rehabilitation, working with those impacted by long-term conditions, and experience of working in a multi-disciplinary team are essential. We are also looking for someone who has a passion for improvement and is looking to contribute to pathway development.
The Symptom Management Service supports people coping with the impact of health conditions in their lives; including Long Covid, Chronic Fatigue, Fibromyalgia, and Chronic Pain. The clinical pathway is focused on working with people to identify and support their needs, in partnership with GPs, psychologists, specialist teams, and community services.
The role will include assessment and interventions focused on meeting the physical, emotional, and social impacts of people with extended health issues. The post holder will deliver interventions to individuals, groups, face-to-face, and remote formats.
The post-holder will also be working alongside voluntary and charitable provisions to link people with others that can help them in their local community.
The successful post holder will be required to respond flexibly to changing needs that emerge as the service develops aligned to the Symptom Management Service, specifically around pain management.
Main duties of the job
The post holder will provide symptom management for energy-limiting conditions and chronic pain management, providing outpatient services as part of an MDT. The role will include:
1. Advanced assessment and treatment of a range of chronic pain conditions and presentations within an outpatient setting.
2. Independent management of highly specialist and/or complex caseload.
3. Responsibility for the clinical guidance, mentoring, development, and teaching of relevant MDT members and students both locally and across the Health Board.
4. Active participation in relevant specialty forums, education, clinical evaluation, audit, and research within the specialty as required.
5. Providing advice to inform service development in their specialty to the Operational Manager and Senior Management team.

Skills and attributes
* The ability to discuss the clinical reasoning process in the management of complex cases.
* The ability to identify and act upon adverse clinical signs.
* The ability to lead and work effectively in a team environment.
* Excellent knowledge of relevant national and local clinical guidelines.
* Awareness of research principles and findings and the impact on clinical practice.
* The ability to support the management of personnel issues.
Experience
* Significant experience at Band 6 with appropriate experience within Chronic Pain Management.
* Significant senior postgraduate clinical experience encompassing complex case management.
* Experience of working in Pain Management Programmes.
* Experience of working closely with medical practitioners.
Qualification and Knowledge
* Diploma/degree in Physiotherapy.
* Registered with Health Professions Council (HCPC).
* Appropriate range of postgraduate clinical education.
* Completion of Clinical Educator's course or equivalent.
* Completion of an appropriate advanced assessment, diagnostic, and treatment course that is recognized/accredited at Masters level.
* Willing to develop competency in Advanced Practice skills.
* MSc or working towards relevant higher qualification.
