Key Responsibilities Co-develop and provide an alternative model to urgent and same day GP home visit for the network. Work as part of an MDT within the PCN; Consisting of Doctors, Pharmacists, Care Coordinators, Social Prescribers, Nursing associates and more. Work within ones own competency to provide patient care which may include triage, assessment, treatment, making any necessary referrals to other members of the primary health care team, other agencies or organise admission if required. Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other services within the PCN, and where appropriate, other community or voluntary services. Be able to perform specialist health checks and reviews within their scope of practice and in line with local and national guidance, perform and interpret ECGs. Perform investigatory procedures as required. Undertake the collection of pathological specimens including intravenous blood samples, swabs, and other samples within their scope of practice, and within line of local and national guidance. Support the delivery of anticipatory care plans and other healthcare to vulnerable individuals as required. Communicate at all levels across organisations ensuring that an effective, person-centred service is delivered. Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required. Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice. Undertake work in line with PCN directed priorities such with regard to health inequalities or current disease prevalence. Enhance own performance through Continuous Professional Development, imparting own knowledge and behaviours to meet the needs of the service. Support remote monitoring when required. Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations. To impart knowledge and skills formally and informally to colleagues promoting peer review and best practice within the work environment. Ensure evidenced-based care is delivered at the highest standards ensuring delivery of high quality patient care. To achieve and demonstrate agreed standards of personal and professional development to meet the needs of the service. To participate in the audit process, evaluation and implementing plans and practice change to meet patient need. Contribute positively to the effectiveness and efficiency of the team and work colleagues. Administration and professional responsibilities. Participate in the administrative and professional responsibilities of the practice team. Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes. Ensure the clinical computer system is kept up to date, with accurate details recorded and amended. Ensure appropriate coding for items of service claims are made accurately, reporting any problems to the practice administrator or PCN Manager. Ensure accurate completion of all necessary documentation associated with patient health care. Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit. Attend and participate in PCN and practice meetings as required. Restocking and maintenance of clinical area, visiting bags as required. Training and personal development Training requirements will be provided and monitored according to HEEs The First Contact Practitioners and Advanced Practitioners in Primary Care: (Paramedic) A Roadmap to Practice guide. As per the roadmap to practice, training requirements will commence for stage 2 following completion of stage 1, stage 2 must be completed in practice within the specified period. Training requirements will be monitored by yearly appraisal and will be in accordance with PCN requirements, including taking responsibility for maintaining a record of own personal and or professional development. Personal development will be encouraged and supported by the PCN. It is the individuals responsibility to remain up to date with recent developments. Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development, ensuring mandatory requirements are met. If it is necessary to expand the role to include additional responsibilities, full training will be given. Develop and maintain a Personal Learning Plan. Post holder may be expected to undertake additional training courses to develop their scope of practice in an effort to provide additional capabilities within the PCN, such as spirometry. As well as the GP and nursing team, there is a need to work closely with admin teams of the member practice you are working with and behalf of to ensure the smooth running of the practice, reporting any problems encountered to the relevant person or to the PCN Manager. Establish and maintain good liaison with all member practices, community services and agencies, including secondary care. Play an important role in building strong relationships for the PCN with external services you come in contact; raising awareness of the PCN and NHS Core Values.
