Job summary The Clinical Pharmacist plays a vital role in supporting safe, effective, and evidence-based prescribing across the practice. Working as an independent prescriber, they conduct structured medication reviews, manage patients with long-term conditions, and provide expert advice on medicines to patients and colleagues. This is a patient-facing role that combines clinical work with wider responsibilities in medicines optimisation, prescribing safety, and service development. The post-holder also supports Care Quality Commission (CQC) compliance, contributes to audits and quality improvement projects, and helps deliver national and local performance indicators such as QOF and IIF.
