We are recruiting for an Advanced Pharmacist - Thrombosis & VTE who share our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.
We are looking for a dynamic, motivated, and forward-thinking clinical pharmacist to work with the Thrombosis Committee, Patient Safety Leads, Quality Improvement team and clinical pharmacy team. The successful candidate will be responsible for providing leadership and delivery of a high quality, proactive and patient focused service to patients relating to Thrombosis and VTE.
This is an excellent opportunity to work with Lead Clinicians and nurses; often at Trust Board level to push the Thrombosis and VTE agenda forward.
You will use your non-medical prescribing qualification to optimise individual patient's drug therapies as appropriate, having the opportunity to develop a clinic.
You will be supported to develop their leadership and management skills.

To be the Advanced Pharmacist for Thrombosis and VTE utilising specialist clinical knowledge to provide a high-quality service to patients within East and North Hertfordshire NHS Trust
Responsibility for leadership and development of clinical services to thrombosis and VTE specialties.
To lead on the Trusts VTE and thrombosis Quality Improvement work and work with the Trust’s Kaizen Promotion Office.
Play an integral role in the multidisciplinary team that delivers a high-quality clinical service to thrombosis patients within the Trust. This includes the undertaking of root-cause analysis for patients who develop healthcare-associated thrombosis; critique the thromboprophylaxis provision they received; feedback results to the relevant Directorate teams and support the preparation & publication of a quarterly report on the data collected.
Develop and maintain a thrombosis and VTE clinic.
Develop haemostasis and thrombosis ward rounds in rotation with other members of the MDT.
Develop, implement, monitor and evaluate protocols and guidelines for anticoagulant therapy. These may be used both Trust-wide and external to the Trust.
To be a clinical mentor and guide junior pharmacists, foundation trainee pharmacists throughout their training programmes.
