An exciting opportunity has arisen within the Review Team.
The Review Team is multi-disciplinary with team members from backgrounds that incorporate Social Work, Registered Mental Health Nursing, and Occupational Therapy, who review individuals who have a funded package of care. Where appropriate, the team will look at lesser supportive packages to enable increased independence.
Main duties of the job
1. Completing reviews of packages of care funded by Manchester City Council and Manchester Integrated Board.
2. Facilitating move on from funded placements where appropriate.
3. Working in a strength-based way, promoting independence and maximizing resources.
4. Working in accordance with key legislation e.g. Care Act (2014), Mental Health Act (1983), Mental Capacity Act (2005), and Human Rights Act (1998).

Job responsibilities
Communication
1. Demonstrate high-level communication skills. Utilize highly developed interpersonal skills including specific skills to facilitate effective communication when there are barriers to understanding e.g. sensory or cognitive impairment or in hostile and highly emotive situations.
2. Establish and maintain robust communication networks with service users, carers, and other health and social care workers and numerous external agencies.
3. Have good written communication skills and able to produce reports for court.
Clinical Care Governance and Quality
1. Work in a strengths-based way, maximizing independence, and promoting wellbeing.
2. Assess complex mental health and social care needs of service users and carers, and the provision in place aimed at meeting this need.
3. Produce a support plan for providers which meets the desired outcomes of the individual but also maximizes independence.
4. Prepare applications to funding panel following review.
Financial and Other Resource
1. Exercise good personal time management, punctuality, and consistent reliable attendance.
2. Maximize access to resources across the provider market and utilize universal services wherever possible.
Learning, Education and Personal Development
1. Participate in regular formal managerial and professional/clinical supervision and a shared appraisal of work performance with the line manager.
2. Engage in continued professional development relevant to role.
Safeguarding Children and Vulnerable Adults
1. Adhere to the principles of the Care Act in relation to the safeguarding of adults with Care and Support needs.
2. Identify safeguarding concerns for children and young people and liaise with Children’s Social Care, Early Help to ensure information is shared and appropriate action taken.
