We are hiring a CASE MANAGER at Abraxas South Mountain Leadership Development Program – starting pay is $22.00 per hour! In this role, you will help improve the lives of at-risk youth in our community.
Abraxas South Mountain is located in Franklin County on Pennsylvania Route 233 near Waynesboro, PA and is surrounded by the rolling hills and beautiful woodlands of south-central Pennsylvania’s Michaux State Forest.

Salary: $22.00 per hour
Shift: 4 shifts, 6:45am-2:45pm and 1 shift 2:45pm-10:45pm, includes weekends.

In this role, you will:
Provide a full range of counseling and case management services to delinquent and/or dependent youth:
* Coordinate and implement case management activities.
* Develop and implement treatment plans including SELF Service Plans, MAYSI, Court Reports, RPP, and other reports as assigned.
* Provide weekly individual counseling and related documentation.
* Participate in, coordinate, and communicate pertinent client information, RPP Meetings, and Sanctuary Meetings for their caseload.
* Write progress/court reports as necessary and discharge summaries.
* Maintain regular contact and communicate effectively with juvenile probation officers/case workers, including them in the treatment process and updating them on clients’ progress.
* Maintain regular contact and communicate effectively with parents/legal guardians.
* Maintain client files in accordance with contractual and licensing requirements.
* Participate in team meetings.
* Ensure clients are prepared for transport to court.
* Ensure accurate billing and statistical reporting for clients on caseload.
* Ensure communication of pertinent information to outside agencies to assist in providing basic needs for clients such as IEPs, prior drug abuse or mental health history.
* Complete client satisfaction index at time of discharge.
* Complete referrals and intake processes.
* Assist with client behaviors and unit operations as needed.
* Support the Abraxas philosophy and mission and promote the Seven Key Principles of care.
* Demonstrate appropriate use of Safe Crisis Management techniques and skills.
