To support In Patient Clinical Lead, Nurse in Charge and Senior Staff Nurses. To assist in developing an environment in keeping with values and behaviours of the Hospice. Demonstrate the ability to develop a greater awareness and understanding of the therapeutic options available to meet the holistic needs of the patients requiring specialist palliative care. Develop a positive, realistic and supportive attitude to ensure the needs of the patient and their families/carers are met. Demonstrate an ability to draw information from the nursing/medical history assessments in order to plan and implement the patients individual care, by setting attainable goals and objectives to help the patient attain these goals. Assist with teaching patients and their families/carers in preparation for changes in treatment, expectations and discharge for home care. Awareness of potential side-effects of any treatment in progress, adopting a problem-solving approach to minimise potentially adverse reactions. Give information, support and advice to families and friends following the death of a patient, referring them to other services/agencies for support if necessary. Communicate effectively with members of the multidisciplinary team, patients and their families.
