Job Summary:
Under the supervision of the Team Manager, the CNA works consistently with identified case managers to provide direct personal care to patients. Communicates regularly with the Case Manager regarding pertinent findings, the effectiveness of the CNA care plan in supporting progress toward outcomes and the potential need for modification to the CNA plan of care.
Duties and Responsibilities:
1. Follows established plan of care, under supervision of the Case Manager, in order to provide para professional patient care to assigned patient population, taking into consideration patient safety, comfort and well being.
2. Makes scheduled patient visits at assigned times. Calls patient or family to schedule and confirm time of visit.
3. Bathes, dresses and undresses patients and assists with personal hygiene to increase their comfort and well being, as outlined in the care plan.
4. Implements household care plan to assist patient with laundry and household chores, changing lines, making occupied and unoccupied beds, meal preparation, feeding patients when necessary, and other activities of daily living.
5. Provides for and assists with elimination needs of patients.
6. Reports/records food and fluid intake and output, as assigned.
7. As directed by the primary professional, reminds patients through 'cueing' to take their required medications.
8. Observes patients and reports unusual reactions or occurrences to the primary professional; reports relevant patient information such as changes in patient conditions, difficulties in maintaining positive patient/family relations, or a need to change schedules, in a timely manner; immediately reports suspected abuse incidents to the primary professional.
9. Obtains vital signs (heart rate, respiratory rate, blood pressure, temperature) under the supervision of the primary professional per CNA plan of care.
10. Sets priorities and organizes work schedules that ensure completion of all aspects of patient and family care on a daily basis; completes day sheet and patient progress notes on a daily basis, in accordance with Agency policies.
11. Documents care provided according to Agency policies; completes Agency paperwork in a timely and accurate manner.
12. Answers patients' general questions with courtesy and concern; refers specific questions to appropriate personnel.
13. Assists patients with physical and occupational therapy exercises, as assigned.
14. Participates in Agency quality improvement activities and committees.
15. Other duties as assigned.
Requirements:
1. High school diploma or equivalent. Continuing education courses as required to maintain CNA certification.
2. Must hold current Rhode Island CNA certification.
3. Must be CPR certified.
4. Must have a valid driver’s license and automobile liability insurance.
5. Minimum one (1) year previous experience as a Certified Nursing Assistant preferred.
6. Previous home care experience preferred.
