Job summary Bridge House Medical Centre is looking for an Advanced Nurse/Clinical Practitioner to join our multi-disciplinary team to replace our highly regarded current ANP who is leaving to take up a more senior role. You would be 1 of 2 highly valued ACPs employed at the practice to provide care to our frail elderly population, some of whom may be housebound or residents of care homes, as well as building your confidence and experience in seeing a wide variety of patients in the practice with minor illness. Main duties of the job At Bridge House Medical Centre you will be exceptionally well supported by the GP Partners, salaried doctors, and wider practice team to assess, diagnose and treat patients autonomously. Your background experience may be varied but we are able to provide tailored training in aspects of clinical care, such as long-term health conditions, end of life care and palliative medicine to support your existing skills. Completion of Respect forms, advanced care planning and liaising with the district nursing team will help ensure seamless care for our patients. During minor illness clinics, you will be able to discuss how your list can be differentiated if desired, so you can best utilise your existing diagnostic skills and further develop your clinical knowledge with the support from the duty doctor and the wider practice team. Whilst out visiting, you will be able to liaise with GPs to discuss patient management whilst also having an established network of support among the wider community ACP team in South Warwickshire, the Frailty Consultants at Warwick Hospital and local palliative care services at Myton Hospice, good relationships have already been established and induction and handover will be robust. Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
