Job summary Occupational Therapist, Ledbury Intermediate Care Unit - 12 month Fixed term - Secondment cover. 18.75 hours per week. We have an opportunity for an Occupational Therapist to join and work along side other members of the multi-disciplinary team based at Ledbury Intermediate Care Centre. This role is a mixture of working in the intermediate care unit and in people's own homes as part of the discharge to assess pathway. The teams aim is to develop optimum levels of function and independence in the physical, emotional and social aspects of life and to facilitate discharge, prevent further admission to hospital or long-term care and to reduce any inpatient length of stay, supporting the principals of care closer to home. To work autonomously on the unit and in the community, liaising with other health and social care professionals, to enable people to facilitate discharge from the community hospital to return to their homes in the community following illness or disability. To provide high quality Occupational Therapy assessments where appropriate and provide direct rehabilitation where necessary managing patients with a range of conditions. Main duties of the job Provide rehabilitative approach to client recovering from acute illness Short term assessment into care facilities. Assessment to manage longer term conditions from an MDT Rehabilitative support for exacerbations of long term conditions Longer-term management plans i.e posture/seating and healthy lifestyles. Signposting/referring to the wider community services (negotiating between ASC, Neuro etc You will need to be able to; triage cases and prioritise cases to effectively caseload manage display an organised and disciplined manner work remotely and autonomously be decisive and a team player - maintaining a good team working rapport daily remotely supervise team members display a passion for community rehabilitation.
