Be able to recognise the deteriorating condition. Complete washing and dressing assessments, kitchen assessments. Refer to social care services as required. Discharge patients from service. Arrange care packages in accordance with protocol. Prescribe exercise and mobility programmes within the scope of practice. Assess level of support required with medication, Administer, Assist, Prompt, None. Participate in the promotion of education and training to patients. Initiate, delegate and progress or adapt treatment plans for non-complex patients. Assist the senior staff with admissions. Assess, provide and fit small aids and equipment within the protocols. Order therapeutic and rehabilitative equipment. Complete access, home, discharge home visits safely. Take responsibility for the safe use of clinical equipment. Actively participate in projects across the Organisation. Other Responsibilities The post holder will be expected to participate in a rota system of shift working to cover a 6- day a week service.
