An exciting opportunity has arisen for a Band 6 Community Complex Care Sister to work in the Continuing Health Care Team.
The role entails managing Band 2 and Band 4 Health Care Support Workers within a specially designated team providing care for a mechanically ventilated patient in his own home.
This role requires an experienced Registrant who will work in partnership with the Team Leader to support and develop a multi skilled health care team to meet the demands of the patient / population within the Swansea area.
The successful candidate will be responsible for providing safe, timely and effective health care provision in conjunction with the Team Leader and other community professionals.
The post holder will lead the competency development of the Continuing Health Care Team and provide training and educational opportunities to facilitate high standards of care delivery.
The successful candidate will be responsible for providing the Team Leader with performance data relating to mandatory training, sickness and absence and PADR compliance and rostering.
The post holder will liaise with all health and social care professionals, social care agencies in primary and secondary care settings.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Main duties of the job
o Work in collaboration with other community professionals; develop measurable outcomes for patient Undertake clinical reviews of patient care to ensure they are receiving the appropriate level of care for their assessed health need or refer to alternative care agencies without delay to ensure flow of care through the Will deputise for the for the Team Leader in their absenceo Provide operational support to health care workers within the team when required, including Sickness reviews, PADR Involving the patient, relative, carers and all agencies in the setting up of a care package where appropriate with the aim of delivering patient centred careo Review patients after discharge, and evaluate and adapt care needs as necessaryo Be responsible for monitoring and improving the community care packages, including auditingo Preventing unnecessary/avoidable hospital admissions through continuous review of patients' needso Work in collaboration with quality outcomes of the QCF framework to support health care workers through education and Provide a quality assurance role with the support of the Medicines Management Team as a Medicines Management Community Assessor within the hub.
