Please refer to the attached job description and person specification for full details of responsibilities Work collaboratively with the team and professional leads to support professional and practice developments including CPFT clinical supervision framework Work autonomously to assess, plan, implement and evaluate programmes of care, providing specialist advice and actively seeking evidence based practice which improves the quality of care and meets the needs of patients. To manage a significant caseload of patients with complex diabetes, managing individual programmes of care working with patients for self management of their diabetes. Provide Community Nurse Led clinics within general practice. Work within the MDT Diabetes Team supporting general practice within virtual Review Clinics. Work proactively to prevent admission to secondary care. To promote and advance non medical nurse prescribing for the benefit of people with diabetes. Develop care plans and treatment protocols for patients with diabetes in primary care. Involvement in clinical audit, present and evaluate said audits. Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act. The post holder must adhere to the Trust Risk Assessment and Risk Management Processes The post holder must adhere to Infection Control Policies and Procedures. It is a condition of your employment that you are currently registered with the NMC and it is your responsibility to maintain your professional registration. Participate in clinical supervision on a regular basis and be responsible for the management of and reporting on individual and team performance indicators and development of remedial action plans as required, The post holder must also participate in clinical and other audits as required.
