An exciting opportunity to work with One Thornton Heath PCN, anestablished primary care network covering six GP practices in North Croydon.This role will be for one year starting in February 2025 to cover maternity leave.
This postrequires working under own initiative with a flexible approach. It willbe instrumental in the delivery of the PCN's aims for patient centred, proactive care andachievement of the PCN targets.
The rolewill require movement between 2-3 Practices and working with amulti-disciplinary team
Main duties of the job
Able to:
Liaise with Practices,CCG, Council, HCOs and VSOs for information, claims etc.
Help to improve PCN cancerscreening uptake and reduction of DNAs.
Help to improve PCNchildhood immunisations uptake by various measures including, but not limitedto, directly calling patients, booking appointments and providing informationto patients.
Work with local systempartners including Public Health, Cancer Alliance, PPGs, ICN, etc.
Supportmaintenance of the PCN practices websites
Lookingfor:
- Highproficiency in all aspects of IT

About us
One Thornton Heath Primary care Network is an established network of 6 GP practices with many years' experience of collaborative working.
All the practices are committed to providing high quality care for the 54,000 registered patients within the PCN. This new post will join the team of four 1TH PCN staff currently working successfully across the practices.
The post holder will be employed by one of the practices as lead employer on behalf of the PCN and the lead practice will provide regular support and supervision, reporting to the PCN Clinical Directors and Business Lead.

CARE COORDINATOR

REPORTS TO: PCN Clinical Directors and Business Lead
Job Summary:
This role is a new role and will evolve with the development of the PCN. The candidate will need a flexible approach to change and be instrumental in the delivery of the PCN’s aims for patient centred care and in particular improvement of cancer screening and childhood immunisation uptake. The role may require movement around the 6 Practices and working with a multi-disciplinary team
Job Responsibilities:
· Proactively identify and work with a cohort of people to support their care.
· Help people to manage their needs, answering their queries and supporting them to make appointments.
· Raise awareness of shared decision-making and decision support tools, and assist people to be more prepared to have a shared decision-making conversation.
· Ensure that people have good quality information to help them make choices about their care.
· Support people to understand their level of knowledge, skills and confidence – their “Activation” level – when engaging with their health and wellbeing, including through the use of the Patient Activation Measure.
· Provide coordination and navigation for people and their carers across health and care services, alongside working closely with social prescribing link workers, health and wellbeing coaches and other primary care roles.
· Support the coordination and delivery of MDTs within PCNs.
Main Duties include:
· Lead on PCN screening improvement plan, with support of practices.
· Help to improve PCN cancer screening uptake and reduction of DNAs.
· Help to improve PCN childhood immunisations uptake by various measures including, but not limited to, directly calling patients, booking appointments and providing information to patients.
· Work with local system partners including Public Health, Cancer Alliance, PPGs, ICN, etc.
· Specific actions to engage with groups with low participation locally in screening and immunisations.

Qualifications
* GCSE grade A to C in English and Maths
Experience
* Experience of working with IT
* Experience of working in a GP practice
