We are looking for an experienced and motivated part time practice nurse to join the team here at St James Medical Centre 27.5 hours per week over 3 days. We are based in leafy Rawtenstall in the valley of Rossendale close to good motorway links.
Main duties of the job
The successful candidate will be part of a team providing practice nursing services to our patient population. Main duties include:
* Managing Chronic Disease.
* Performing Cytology Testing.
* Running vaccine clinics such as childhood vaccines and seasonal vaccines.
* Performing monitoring tests such as BP, ECG, Spirometry, and blood tests.
* Infection Prevention and Control.
* Involvement in achieving QOF targets and other targets as required.

We have the benefit of a number of ARRS staff clinics such as PA, TNA, Mental Health Practitioner, First Contact Physio, and Health and Wellbeing coach. We are a training practice and this fosters a culture of support for our staff. We have excellent clinical and administration teams and effective processes in place to help clinics run smoothly.
Job responsibilities
Assess, plan, provide, implement, and evaluate nursing care to meet the care needs of individuals in the practice population.
Provide access to specialist nurse/GPs.
Identify and manage treatment plans for patients at risk of developing a long-term condition.
Prioritise health problems and intervene appropriately to assist patients in complex, urgent, or emergency situations, including initiation of effective emergency care.
Check all emergency equipment and drugs regularly and ensure records are up to date.
Support patients to adopt health promotion strategies that encourage healthy living, and apply principles of self-care (e.g., diet, exercise, alcohol, smoking cessation).
Actively maintain health promotion material within the practice and waiting room notice boards.
Undertake health screening, surveillance, new-patient screens, and immunisation.
Maintain patient confidentiality and accurate record keeping.
Attend and participate in practice team and clinical meetings.
Vaccination and immunisations, administering injections, ensuring safe vaccine storage, implementing cold chain policy.
Infection Control: support patients and colleagues in adapting sound infection control measures, implementing infection control policy.
Mentor and assess trainees and junior team members.
Specific Health Care Responsibilities
Child Care and Child Protection
* Recognise signs, symptoms, and categories of child abuse.
* Understand the importance of recognition, observation, documentation, and communication.
* Be aware of child health procedures, statutory local procedures, and points of referral.
* Have knowledge of other practitioners' and agencies' roles.
* Understand basic legal issues around child abuse.
Family Planning
* Advise patients on available methods of contraception and refer as appropriate.
* Support patients attending for emergency contraception and refer as appropriate.
* Perform interval health checks on patients attending using hormonal contraception.
* Teach patients on the risk of STDs and refer as necessary.
* Advise women on national/local cervical cytology and mammography screening programmes.
* Undertake cervical smears.
* Audit a minimum of 20 consecutive smears each year to demonstrate adequate sampling rates.
* Promote breast self-awareness and provide supporting literature for breast self-examination.
* Take swabs for microscopy as directed.
* Make appropriate referrals regarding menopausal symptoms, osteoporosis, and heart disease.
* Advise on basic incontinence management and make appropriate referrals.
Chronic Disease Management
* Assist in maintaining GP contract regarding chronic disease patients.
* Work within practice guidelines, ensuring safe practice and acknowledging limitations.
Travel Health Advice
* Vaccinations and medications.
* Malaria prophylaxis and bite avoidance.
* Safe sex.
* Food and water hygiene.
* Health insurance.
* First aid.
Other
* Any other tasks deemed reasonable and necessary to the role.

Person Specification
Experience
* Experience of managing Chronic Disease including Respiratory, Heart, and Diabetes.
* Experience of running vaccine clinics including childhood vaccines and seasonal vaccines.
* Experienced in taking cytology tests.
* Meeting QOF targets.
* Experience of supporting junior team members.
* Experience of using EMIS.
