The Role
In this role you will manage and monitor all aspects of the children services and adult services, which include all services such as: domiciliary care, personal assistant (PA), community-based, day opportunities, and centre-based care. These services are provided during weekdays, evenings, weekends, and holidays.
Duties and Responsibilities
• Process referrals and carry out assessments for all domiciliary care, PA, community-based, day opportunities, and centre-based packages in line with council contractual obligations and CQC regulations.
• Create and implement care plans, risk assessments, and other relevant documentation via the care planning system, ensuring they are designed and personalized around the assessed needs of the child/young person/adult.
• Be the first point of contact for all existing and new referrals, i.e., child/young person/adult and care workers.
• Oversee the activity program for the day opportunities and community-based service, ensuring activities are designed and personalized around the needs of the individuals and that staff are following the outcomes and encouraging/prompting the adults to take part in positive and meaningful activities.
• Ensure a suitable match between the child/young person/adult and care worker.
• Carry out spot checks to ensure quality care and standards are being met. Identify potential issues with children/young people/adults within their homes where care is being provided, which may put staff at risk, and ensure issues are reported to the Registered Manager to resolve the issue/risk.
• Attend all reviews for children/young people/adults receiving services and liaise with families and other professionals as appropriate.
• Review, update, and manage care plans on the care planning system to ensure quality of service and achievement of individual outcomes to meet CQC Regulations.
• Carry out all risk assessments for each child/young person/adult and for each activity (in-house and external) as appropriate and necessary, including provision of toileting/personal care facilities and evidence staffing requirements (e.g., 2:1 staffing).
• Manage safeguarding by implementing internal procedures and controls to safeguard vulnerable children/young people/adults and take remedial action as necessary.
• Manage staff by undertaking probation reviews, supervisions, annual appraisals, and giving access to regular support, as well as monitoring and managing performance in line with council contractual obligations and CQC regulations.
• Facilitate meetings as appropriate with staff and the services team.
• Initiate absence management procedures, informing HR of any staff absences.
• Ensure care policies and procedures are recognized, understood, updated, and maintained across the company, as well as ensuring that staff act in accordance with those policies and procedures as well as statutory requirements.
• Liaise with the administrator to ensure the monthly rosters cover all care packages.
• Gain sufficient knowledge to operate the rostering and scheduling system, ensuring adequate staff cover, as well as authorizing annual leave for care staff and cover for those shifts.
• Add, update, and oversee the individual child/young person/adult details on the Care Planning system.
• Ensure adequate staff cover is available where an allocated care worker is unable to work a shift; the Care Coordinator will need to ensure they match the individual to the next best matched care worker.
• Cover the out-of-hours on-call support on a rota basis.
• Keep up to date and report on any compliance changes to the CQC regulations where applicable.
• Manage incidents and complaints and take remedial action as necessary.
• Represent the company as required at stakeholder meetings.
• Liaise with other functional/departmental managers to maximize communication.
• Organize in-house mandatory training for staff.
• Encourage new ideas, maintain staff morale, and promote high standards and development of skills within the team.
• Assist in the inductions for new employees.
• Maintain audits, quality checks, performance monitoring in conjunction with the Registered Manager prior to and during audit and inspection visits.
• Participate in the recruitment and selection process as required.
• Produce an annual report including summative data for the year and a commentary on the service, as well as a 6-month operation report and reports for the Board.
• Carry out any other duties reasonably required of the post.
