Independence Advisor - Hospital Discharge Team We have an exciting opportunity for an Independence Advisor role within the Hospital discharge team. About the team: In Southampton, we face the challenges familiar to local authorities across the country. We’ll have a team from Newton working with us for the next year to support us deliver our plans on track. In September, we’re changing the way we work to strengthen our approach and ensure that we work with our partners to keep residents healthy, independent, connected and contributing to society. This position requires working on weekends (one weekend every month) and includes a 10% allowance. The team is dedicated to supporting the safe discharge of Southampton residents from acute hospital settings through collaborative problem-solving and close partnerships. Despite the challenges posed by competing priorities and demands, the team strive to ensure that discharges are timely, safe, and person-centred.
