West Kent Primary Care is looking to recruit an Advanced Clinical Practitioner (ACP) on behalf of Tonbridge Primary Care Network (PCN). Serving approximately 70,000 patients; TonbridgePCN strives to develop collaborative working across Tonbridge GPpractices and local health and care services.
This is a 12 month fixed contract, with a view to extend.

TonbridgePCN are seeking to recruit an Advanced Clinical Practitioner to complement ourgrowing PCN team of paramedics, pharmacists, and care coordinators to supportour practices in the delivery of urgent care. We are looking particularly foranACPwho isconfident in triage, management of 'on the day' primary care and communicationwith GP and MDT colleagues in the ongoing follow up and treatment of patientsas required.

Tonbridge recently won a Kent and Medway award for itsIntegrated Home Visiting Service, jointly staffed by PCN GPs, paramedics andlocal community nursing teams.
The PCN aims to support and strengthen delivery of primary care,offering high standards of patient care centred on the needs of our population,whilst working to support a happy and resilient workforce through thedevelopment of robust multi-disciplinary teams. A Same Day Access PCN Hubservice with GP supervision using ACP clinical skills is currently indevelopment, and we hope this will not only support patient access but providea greater range of clinical experience for ACPs working at PCN level.

Clinical practice
The role willmainly involve working in our Same Day Access Hub seeing patients for urgent onthe day issues. The role will involve working alongside PCN team of GP supervisor /Paramedic Practitioner in the PCN Same Day Access Hub seeing patients on theday with urgent issues.
The post may include cross cover with the award-winningFrailty Visiting Team visiting patients who are housebound for urgent on theday issues.
Work as part of a multi-disciplinary team todeliver safe and high-quality care.
Carry out clinical practice at an advancedlevel using expert clinical skills, critical judgements & knowledge todeliver appropriate and safe care.
Assess, diagnose, plan, implement and evaluatetreatment/interventions and care for patients presenting with acute, long termand complex health needs.
Manage undifferentiated undiagnosed conditionand identify red flags
and underlying serious pathology and takeappropriate action
Triage and treat patients, including same daytriage, performing assessment of their needs and makes referrals as necessaryto other members of the primary and secondary health care teams as appropriate.
Interpret investigation findings to determineeffective treatment plans & discuss with GPs where a second opinion isrequired
Proactively identify, diagnose and managetreatment plans for patients at risk of developing a long-term condition, usingboth non-pharmacological and pharmacological treatments.
Co-ordinate the planning and delivery of careensuring patients and carers or relatives are involved where required. Activelytake a personalised care approach and population centre care approach to enableshared decision making.
Prescribing and reviewing medication that isappropriate for patient needs and in accordance with latest evidence-basedpractice, national and practice protocols and within the scope of qualifiedpractice.
Diagnose and manage acute and chronicconditions, including drug and non-drug based treatment methods into amanagement plan.
Request diagnostic investigations andinterpret within agreed protocols x-ray, blood tests, scans, etc.
Assess, diagnose and treat patients inpractice premises or their own home who require medical attention and/or referto appropriate agency.
Refer patients to other members of themulti-disciplinary practice teams, secondary care organisations and otherorganisations as necessary.

Uses and adapts communication style to meetindividual patient needs to ensure patients are fully informed about treatment.
