Job Title: Social Worker - Older People's Mental Health (OPMH) Team
Location: Bath
Pay rate: Up to £38 per hour
A local Council is seeking an experienced, hardworking, and reliable Social Worker to join their Older People's Mental Health (OPMH) team.
What will your responsibilities be?
In this role, you will provide an outcome-focused assessment service to adults in line with the Care Act 2014. You will deliver a professional social work service, including statutory and legal social care responsibilities to members of the public and/or their carers who require an assessment of their needs. You will work closely with other teams, including undertaking joint assessments with providers, carers, and external agencies to deliver outcome-focused services to individuals and their families. You will coordinate a comprehensive holistic assessment of need and risk, establishing and reviewing an initial support plan agreed with the service user, carer(s), and service providers. Additionally, you will participate in the department's safeguarding adults' procedures and fully comply with departmental policies and procedures and the Health and Care Professionals Council's Code of Conduct for Social Workers, including the capability framework.

Qualifications and Experiences:
To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
