Job summary Ashfield Medical Centre have an exciting opportunity for a practice nurse to join the team on a part-time basis for 14 hours per week. These hours will ideally be worked over two days; on a Monday, Tuesday or Friday. Working alongside an experienced multi-disciplinary team, and in accordance with specific practice guidelines and protocols, the practice nurse will assist the practice clinical team in the provision and delivery of patient care. Main duties of the job Undertaking health assessments, administering medication and treatments, and performing diagnostic tests. Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met. General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
