Job Opportunity: Qualified Social Worker - Hospital Team
Location: St Richards Hospital, Spitalfield Lane, Chichester PO19 6SE
Pay Rate: £35 per hour (plus £150 weekly expenses for candidates living outside West Sussex)
Role Overview
West Sussex is seeking a dedicated Qualified Social Worker to join the Hospital Social Work Team at St Richards Hospital. The team works collaboratively to assess and support individuals during and after hospital stays, ensuring their independence and well-being.
Key Responsibilities
1. Assessments: Conduct strengths-based assessments for individuals in the hospital and post-discharge.
2. Planning: Develop and implement effective care plans in collaboration with carers, colleagues, and external agencies.
3. Family Meetings: Attend family meetings and provide clear advice and guidance.
4. Safeguarding: Undertake and contribute to safeguarding enquiries.
5. Support Independence: Work to help individuals regain skills and independence post-hospitalization.
6. Multi-Disciplinary Collaboration: Engage with professionals in A&E, emergency floors, and the voluntary sector to provide holistic care.

Experience and Skills Required
Essential:
1. Minimum 2 years' experience in social work.
2. Strong skills in assessments, care planning, and risk enablement.
3. Familiarity with safeguarding practices and legislation.
4. Proficiency in IT for record-keeping and data management.
5. Effective communication and interpersonal skills for working with diverse stakeholders.
Desirable:
1. Experience in hospital or health-related social work settings.
2. Ability to mentor and support less experienced colleagues or students.
Working Arrangements
1. On-Site Requirements: Minimum 2 days per week at St Richards Hospital for face-to-face assessments and duties.
2. Flexible Hours: Align work schedule to support patients' needs.
