We are currently looking for a motivated and committed Practitioner who would like to have experience in managing the team and service.
The Frailty Hospital at Home service supports both early discharge home of patients and admission avoidance. Patients are hospital level patients, who would otherwise be in a hospital bed and are receiving daily interventions at home for 1-14 days.
The Hospital @ Home ward operates from a base in Watford, but covers all 4 South and West Hertfordshire localities. You will need a full/valid UK driving licence and have the use of your own car daily to visit patients in their own homes.
We are a multi-disciplinary team consisting of a Nurse Consultant, GP, Advanced Clinical Practitioners, Enhanced Clinical Practitioners, Senior Nurses, Physiotherapist, and Technical Instructors with administrator support.
The post holder will be an autonomous practitioner, collaborating with colleagues in primary care, community, and hospital settings as appropriate.
The Post Holder Will Be Responsible For:
1. Leading, motivating and supporting team members in the facilitation of professional and clinical development.
2. Undertaking comprehensive health assessments, physical examinations and ordering tests for patients on the pathway.
3. Using advanced clinical decision making and assessment skills, coordinating care planning and initiating referrals to diagnostics for appropriate tests.
4. Establishing, delivering and leading a comprehensive service within community services.
5. Playing a leadership role within the team and working with external stakeholders such as London Ambulance Service and Social Services to develop new ways of working and increase the number of referrals from the acute and community.
Candidates should be an experienced nurse with sound clinical knowledge and proven experience in acute and community clinical environments. You will need to be adaptable to operate in a fast-moving environment and have the clinical skills and ability to function as a lone worker in a larger team.
Candidates should have excellent communication skills; and the ability to provide care as determined by the clinical assessments. Candidates will have undertaken professional development and have extended nursing skills, with evidence to support.
