We currently have a vacancy within our service and are looking for motivated and enthusiastic Learning Disability Nurses to join our team.
The role involves assessing eligibility criteria under the NHS National Framework for Continuing Healthcare and Funded Nursing Care 2009 (revised 2022), planning and commissioning packages of care (including Personal Health Budgets), supporting individuals, their families, and health professionals through the Continuing Healthcare process (including transition from child to adult services) and reviewing eligibility/packages of care.

Main duties of the job
We need a nurse practitioner who is experienced in working with people who have learning disabilities - an essential characteristic.
NHS Continuing Healthcare (CHC) is the name given to a package of care that is arranged and funded solely by the NHS for individuals who are not in hospital, but who have complex ongoing healthcare needs.
Devon Partnership NHS Trust is responsible for the decision-making and commissioning processes, working closely with the Local Authorities on behalf of Devon ICB.
Successful candidates will have excellent communication and assessment skills; be resilient, able to work autonomously but also work as part of a team. This is a community-based post and although the role is based in Mid Devon, post holders will be expected to travel covering the whole of Devon, and occasionally out of the county.
The team values a role which is fast-paced and employs the use of holistic nursing where we look at the needs of the person. The nature of the role is autonomous and challenging, but the benefit of supportive colleagues.

To actively engage with a wide range of stakeholders including staff, service users, their carers, and other statutory and voluntary sector agencies to ensure people are fully briefed, know, and understand commissioning and care management processes in relation to Continuing Healthcare, Funded Nursing Care, health and social care funded packages of care, and section 117 aftercare.
Impart complex and sensitive information to:
* Service users and families/carers regarding assessment, care planning, treatment, and review.
* Multi-disciplinary teams to ensure the provision of consistent well-coordinated care.
* Primary care teams regarding client care.
* External agencies involved in the provision of care e.g., voluntary and private sector, police, social services, housing, employment, etc.
* Safeguarding, DOLS teams, Clinical Commissioning Groups, and Devon County Council regarding concerns arising from commissioned packages of care.
* Liaise with a wide range of other professionals and agencies.
* Contribute to the safe and effective day-to-day operations of the service including the handling of clinical inquiries.
Analytical and Judgement skills
* Use clinical judgement to support the procurement and commissioning of packages of care for adults with a learning disability and complex health needs to ensure people are maintained within their local communities, avoid inappropriate admission to hospital, and facilitate timely discharge from inpatient settings.
* Use clinical judgement to consider complex facts or situations, requiring analysis, interpretation, and comparison of a range of options when determining the type of care package required for someone with complex health needs.
Planning and Organisational Skills
* Organise and facilitate complex multi-disciplinary team meetings, including DST review meetings and Section 117 aftercare meetings.
* Plan and organise individual packages of care and ensure that reviews are held within the required timeframes, for example CHC, FNC, and Section 117 annual reviews.

* Registered Nurse - with Learning Disability experience
* Postgraduate qualification relevant to specialism

* Ability to be mobile across a geographical area to meet the demands of the role
Knowledge and skills
* Sound understanding of the Continuing Healthcare Framework, including Funded Nursing Care and Section 117 aftercare.
* Knowledge and understanding of Mental Health and Mental Capacity Acts.
* Excellent clinical assessment skills.
* Ability to work alone and as part of a team.
Experience
* Demonstrates experience of effective multidisciplinary and multi-agency work.
* Experience of working with people with a Learning Disability with complex physical health, challenging behaviour, and/or mental health needs.
