A bit about the role Clifton Road care service supports young people 16- 21 years of age who have been previously looked after in care i.e. local authority children homes, foster care, secured residential homes. We provide accommodation and support in a residential setting with a range of services to meet individual needs in partnership with other agencies to improve the life chances of young people and help support independent living. As the Team Leader you will spend time supporting the staff team through supervision, team meetings and running a live rota. You will also spend time working directly with the young people within the service following detailed care plans. How you'll help to create brighter futures Managing and leading a team of residential professionals and other staff as directed in the delivery of a 24 hour rota (including management of shifts) of care, utilising Action for Children policies and procedures to enable staff to provide a responsive, high quality person centered service. Building and maintaining positive relationships by providing emotional/social support to young people in a therapeutic way, who can display challenging emotional and social difficulties Taking the lead with case/care planning management for individual children or young people, undertaking assessment of need, developing a care plan and identifying agreed outcomes and carrying out regular review in conjunction with children, young people and their families and other professionals. Supervise, guide or direct staff and volunteers where required and provide mentoring to newly appointed staff Attending professional meetings including reviews, court work etc. referring appropriate cases and issues that require the involvement of a specialist, other management support or to directly provide the specialist support. Let's talk about you SVQ4 Leadership & Management and HNC social care qualification. The ability to work shifts on a rota basis and participate in the on call rota. SSSC registration. Experience of planning, prioritising and evaluating activities and own work effectively The ability to demonstrate informed decision making and judgement, utilising these skills to anticipate and identify problems and find solutions.
