Location: The Community at Rockhill- 3250 State Rd, Sellersville PA, 18960
Schedule: 7am-3pm, 3pm-11pm or 11pm-7am including an every other weekend and an every other holiday commitment.

POSITION SUMMARY:
Provide personal care services for the residents in assigned area, including activities of daily living and other duties in the Personal Care area. Must respect and show concern for all residents regardless of age, race, religion, gender, nationality or mental status. Must demonstrate an appreciation of the heritage, values and wisdom of the residents and an understanding of the aging process.
SPECIFIC DUTIES AND RESPONSIBILITIES: Personal Care Assistant
* Ability and willingness to work at scheduled times and is flexible to adjust to changes in routines and procedures as facility grows and changes.
* Ability to establish relationships with residents and their families.
* Ability to plan and complete daily assignments with minimal direction.
* Willingness to attend and participate in staff meetings, in-service and ongoing education.
* Assist in a variety of tasks related to dining, i.e., escorting residents to dining room. Assist with serving meals; assist with table set up and clean up, etc.
* Provide assistance with housekeeping tasks such as vacuuming, emptying trash, bed making, getting soiled linens and laundry to appropriate locations and restocking clean linens and putting clean personal laundry in the appropriate location.
* Ability to carry out the following physical functions adequately: balancing, climbing, crouching, grasping, kneeling, lifting, pulling, pushing, reaching, standing, stooping, sitting, and walking.
* Ability to be exposed to employ the following sensory and communicative activities essential to this position; feeling, hearing, seeing, smelling, speaking and tasting.
* Ability to be exposed to inside and outside environmental conditions.
* Ability to function properly as per instructions given under inside environmental conditions possible exposure to blood borne pathogens and chemical hazards.
* Ability and willingness to complete other responsibilities as directed by Director of Personal Care and PC Nurse.
* Adheres to the Values and Expectations statement and element of the compliance program.
SPECIFIC DUTIES AND RESPONSIBILITIES: Med Tech
* Assist with medication administration and treatments as ordered.
* Provides hands-on care of residents as written on the Resident’s Service Plan and Support Plan.
* Observes and reports all pertinent information, including vital signs, abnormal conditions and changes from previous conditions to the Charge Nurse.
* Responds to call bells promptly and courteously.
* Keeps assignments, routines and support plans up-to-date.
* Assists residents with dining, tray delivery, and offers nourishments/liquids to residents at mealtime, between meals and when needed.
* Maintains resident rooms, support centers, and living rooms in a neat and orderly appearance.
* Uses equipment properly, keeping it clean and in the proper place.
* Must have and maintain current CPR certification.
* Maintains First Aid Certification.
* Adheres to dress code required for role.
* Is dependable, competent, observant, and accurate.
* Fulfills tasks as outlined specifically in Procedure Manual.
* Adheres to policies regarding confidentiality, HIPAA regulations, and resident and client rights.
* Complies with regulatory agency standards.
* Complies with safety and infection control protocols in all aspects of the job.
* Demonstrates understanding of the Code of Conduct and abides by its guidelines in all aspects of the job.
* Attends in-services and team meetings as required for role.
* Additional duties and responsibilities as assigned.
* Functions as a positive team member in the development and encouragement of relationships that supports community, both interdepartmentally and intradepartmentally, with an Honoring Lives philosophy of serving others.
* Demonstrates flexibility and openness to learning and adapting to change as roles and expectations evolve over time.

POSITION REQUIREMENTS:

Education: Must be 18 yrs old and have a high school diploma and/or equivalent.
Experience: Completed direct care staff training and competency test.
Professional Membership, Licensures, Certifications: Medication Administration trained with certification- Preferred.
