We are looking for an individual with enthusiasm for change and service development as we build the key components of an integrated system of care for frail elderly patients. Main duties of the job This post will support the delivery of an acute frailty service and will provide clinical leadership in the strategic development of the service which will further enhance the care and improve the care provision for frail elderly patients in North Warwickshire. The GEH Elderly Care team is an innovative, enthusiastic and multidisciplinary team consisting of: 5.0 wte consultants; Dr James Egbuji, Dr R Mappilakkandy, Dr J Alamgir, Dr Mukundun, Dr Ahmed 2.0 wte Specialist Registrar /Trust middle grade doctors 3.0 wte rotational FY1s 2.0 wte rotational SHOs 4.0 wte ACPs 3.0 wte Clinical Nurse Specialists 3.0 wte Physician Associates In addition the team is well supported by therapies in all of the services provided. Inpatient care is provided on Bob Jakin (18 beds), Victoria (27 beds) and on AMU as part of the frailty service. In addition, the team will operate from a state of the art frailty assessment unit (FAU) which allows evaluation of the patient and avoidance of admission, or if this is not possible, early care and discharge planning to mitigate the risk of Hospital Acquired Functional Decline and extended length of stay. This post will further enhance that care and provide opportunities to extend the service and remit. Within the service it is anticipated that the consultant for this new role will have an average caseload of 14 patients during ward rounds.
