Job summary This is an exciting opportunity for someone with prior knowledge of tissue viability to take on a secondment role for 9 months.The aim is to broaden your skill set and understanding of caring for patients with complex wounds, whether in their own environment or in a hospital setting. The position will support the existing Tissue Viability Team in monitoring and evaluating wound care services across all care areas within LPT. The ideal candidate will demonstrate experience in delivering wound care training programs. We are looking for someone who possesses: - A strong commitment to working collaboratively across all settings and supporting MDT development An ability to promote patient independence Sound theoretical knowledge underpinning highly skilled assessment and nursing skills A strong understanding of wound care, evidence-based practice, and integrated working within a culturally diverse population. You will receive regular clinical & managerial supervision and be supported by an experienced team. Additionally, there will be opportunities for flexible working. Post-registration experience and completion of courses in wound care, leg ulcer care and pressure ulcer prevention are desirable. Previous experience working at Band 6, along with knowledge of managing patients with complex needs, would be beneficial. Leading and coordinating the assessment of individuals' and the team's learning needs across the community nursing teams. Leading reflective sessions for teams involved in incidents relating to Tissue Viability. Implementing and monitoring the implementation of policies, standards and guidelines across the LPT nursing teams where applicable. Contributing to the development of policies, standards and guidelines for tissue viability. Supporting the teams to improve practice to meet patient's health needs, improve quality and manage risk. Assessing patients with complex wound care needs or who may require specialist interventions in a range of settings. Referring patients and carers to other healthcare practitioners when there is a need for more specialist assessment or care including direct referral to the acute sector under agreed protocols. Undertaking technical nursing treatments for patients with complex/specialist healthcare needs. Reassessing patients with complex wound care needs or those who require specialist interventions, evaluate the effectiveness of care plans in meeting those needs and improve care plans accordingly. Ensuring that all teams have access to a fully trained knowledgeable link nurse for wound care.
