We are seeking a dedicated and compassionate Adults Social Worker to join our Locality team at a Grade H level. In this role, you will be pivotal in ensuring that adults in our community receive the support they need to lead fulfilling lives. You will work closely with individuals, families, and multidisciplinary teams to assess needs, create care plans, and provide resources that promote independence and wellbeing. Your expertise will not only help individuals navigate complex social systems but will also empower them to make informed choices regarding their care. As an Adults Social Worker, you will engage with various stakeholders, including healthcare providers, housing services, and community organizations, to deliver coordinated and effective interventions. You will be expected to maintain up-to-date knowledge of local resources and legislation affecting adult services and advocate for the best outcomes for your clients. This role requires strong communication skills, empathy, and the ability to work in a fast-paced environment. You will play a crucial role in transforming lives, making a meaningful impact in the community, and ensuring that all adults have access to the services they need.
Responsibilities
Conduct comprehensive assessments to identify the needs and strengths of adults requiring support.
Develop, implement, and review personalized care plans in collaboration with clients and their families.
Assist clients in accessing appropriate community resources, services, and supports to enhance their wellbeing.
Facilitate multi-agency meetings to promote coordinated care and intervention strategies.
Provide emotional and practical support to clients and their families during difficult times.
Advocate for clients' rights and services, ensuring they receive the assistance they require.
Maintain accurate and timely records of client interactions, assessments, and care plans in accordance with regulatory standards.
Requirements
A relevant degree in Social Work and registration with the appropriate regulatory body.
Demonstrable experience working with adults in a social work setting, ideally in a locality team.
Strong knowledge of relevant legislation, policies, and procedures in adult social care.
Excellent communication and interpersonal skills to build rapport with clients and colleagues.
Ability to work independently and as part of a multidisciplinary team in a high-pressure environment.
Strong organizational skills and the ability to manage a varied caseload effectively.

Start Time: 07:30
End Time: 16:00
Pay Per Hour: £18.00
