Job summary
An exciting opportunity has arisen in the North Urgent Community Response (UCR) team for an Occupational Therapist to join us in delivering high quality therapy interventions to patients in their own homes.
The North UCR team provides effective short term reablement/rehabilitation programmes for vulnerable patients with acute health needs and responds to patients in crisis to prevent unnecessary admission to hospital.
This position would be ideal for a professional looking for a secondment role to gain experience in an admission avoidance service and developing the skills and knowledge associated with having shared professional competencies to promote a multi-disciplinary approach. The team is comprised of unplanned nursing and therapy staff and we will shortly be implementing a North Virtual Ward service in the autumn of 2024.
It would be an expectation for the post holder to be involved in thetraining of other team members to maintain competence within some aspects of Occupational Therapy, as well as, develop and maintain own competencies in areas of Physiotherapy and Nursing. The role will predominantly involve face to face intervention, with an element of Triage for the Urgent Community Response Team on a rota basis.

1. To demonstrate a flexible approach, being able to adapt and respond effectively and efficiently in potentially stressful case scenarios.
2. To use use their Occupational Therapy skills within their assessment to determine need for further investigations and to support the Urgent Community Response Team within ongoing patientmanagement in order to prevent avoidable admission to hospital.
3. To lead in the delivery of high quality Occupational Therapy interventions to patients within their own place of residence to prevent avoidable admissions to acute care.
4. To provide short term programmes of reablement/rehabilitation with effective short term goal setting for vulnerable adults with acute health needs. This will involve using health coaching skills to effectively engage with the patient and their family to ensure that expectations are appropriately managed.
5. To show a level of resilience by working with the team to develop competencies in all areas of practice. I

The Urgent Community Response team provides a seven day service commissioned to provide an MDT approach to prevent unnecessary hospital admissions and to support timely discharges from acute and community hospitals. The team consists of Physiotherapists, Occupational Therapists, Nurses, Palliative Care Specialists and Assistant Practitioners, working closely with Norfolk First Support as part of the Network of Escalation Avoidance Teams (NEAT) and Discharge to Assess facilitation pathways.

The post holder will use their therapy diagnostic skills within their assessment to determine need for further investigations, and to support the MDT with ongoing patient management.
Main Duties & Responsibilities. As part of the Urgent Community Response (UCR) Team you will be required: To work within the Urgent Community Response Team to preventunnecessary admission to hospital. To make risk assessments, establishing the safety or otherwise of a patientsability to stay at home, or return safely from another care setting, providingand arranging as necessary supportive packages of care. To provide evidenced based clinical/therapeutic interventions based on bestpractice in order to improve health outcomes and promote choice. To work within the Urgent Community Response Team to assess andprescribe care pathways for patients in a crisis situation with long termconditions, the frail and elderly, palliative and end of life care and/orrehabilitation needs, to achieve quality of life and independence wherepossible. To provide in-reach to inpatient units and step up beds where required. To act as the patients advocate, facilitating choice and patient empowerment. To work with all health care professionals, and statutory/non statutoryagencies to provide a seamless, integrated service to our service users. To Triage referrals on a rotational basis for the Urgent Community Response Team.
Clinical. The post holder will:1. Undertake comprehensive occupational therapy assessments of patientsincluding those with a complex presentation, using investigative, analytical and clinical reasoning skills, and formulate a graded treatment Formulate and execute individualised occupational therapy treatment plans for patients including those with complex presentation, in collaboration with the Urgent Community Response Team, other agencies and the patient, utilising a wide range of treatment Provide assessment of functional ability and safety of patients in thecommunity, to identify care, equipment, adaptation and therapy Assess and plan treatment including teaching patients to utilise techniques and equipment to maximise their functional Be aware of Assistive Technology and make referrals for this as Assess patients holistic needs, communicating complex and sensitiveinformation to patients and carers as to their assessment, diagnosis, prognosis and treatment Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and Communicate effectively with patients and carers to maximise rehabilitation and treatment potential, and to aid motivation and understanding throughout the treatment Establish and maintain therapeutic relationships with patients and carers, incorporating motivation, encouragement and confidence building to enable patients to engage in their treatment/care/management with GPs, Social Services, inpatient teams, other allied health careprofessionals and the voluntary sector to ensure identified needs are met and care co-ordinated participate in multi-disciplinary/multi-agency meetings as appropriate, Gold Standard activity data and clinical information is recorded on Systm One and is completed on a daily and organise specialist services within the Urgent Community Response dexterity and co-ordination when using specialist equipment and fine tools, manual and mobilising equipment and supplies via electronic ordering systems for use within the informed consent is obtained prior to initiating be exposed to bodily fluids, infected material, blood products on a daily basis, therefore must utilise universal precautions and adhere to infection control policies.
