GP Healthcare Alliance (a federation of GP practices in Castle Point and Rochford) is working in partnership with the local Primary Care Networks to support the delivery of the NHS England, Direct Enhanced Service (DES) contract. We are looking to recruit a data and outreach coordinator/care coordinator to join a wider team based at the PCN Office in Canvey Primary Care Centre, Canvey Island.
Main duties of the job
* Minute taking, meeting organisations, and following up with actions.
* Producing data reports and audits from SystmOne.
* Acting as the first point of contact for the clinical team to provide support with general queries.
* Maintaining the staff clinical rotas on SystmOne.
* Providing administration support to the PCN educational events.
* Working with the clinical team with scheduling outreach events and forums.
* Liaising with patients and members of the community.
* Supporting the PCNs Patient Participation Group (PPG).
* Participating in community outreach events to raise awareness of national/local health campaigns to promote health and wellbeing to the local population.
* Support the coordination and delivery of the PCN led Multi-disciplinary Team (MDT).
* Work closely with all relevant care agencies to ensure a coordinated patient care plan.

Person Specification
Experience
* An experienced, effective administrator, with excellent IT, time management and organisational skills.
* A confident individual who can communicate with a wide range of professionals at all levels.
* The successful applicant will have the ability to work without supervision using their own initiative, organising and prioritising own and other workloads to changing and often tight deadlines.
* Previous primary care experience is required.
* SystmOne experience is essential.
