To provide advice, support and information to service users and carers External to Trust Liaise with other agencies, providers, service users/carers during the planning implementation and evaluations of care. The post holder with senior staff and on occasion without senior members, participate in training/education to service users/carers and other teams in the LYPFT and Voluntary Sector organisations who want to know about Acute Inpatient Services. Reporting and Recording Post holder will maintain and produce detailed contemporaneous and accurate records and reports at all times in accordance with agreed procedures. Assist with data collection and the daily completion of statistical information Use of computer to aid work. Contribute data for local performance indicators. Follow integrated care pathways as specified for Acute Inpatient Services and to report on variances and feedback as necessary. The post holder on occasions may be involved in project work or contribute ideas/participate in sub groups to provide feedback on specific topics that they currently have experience with in their scope of professional practice. 1. Knowledge, training and experience Registered Occupational Therapist. Post qualifying experience and knowledge of the problems and difficulties experienced by people with mental health illness. Knowledge to advise and support local authority staff and other professionals in the day service setting in relation to health and recovery related issues. Insight into multi-disciplinary working including working with the voluntary sector. Demonstrate knowledge in person centred approaches and awareness of legislation that affects delivery of health care in mental health services. Demonstrates experience in assessing and formulating treatment plans that are recovery focused. Demonstrates importance of sound evidence based occupational therapeutic health interventions when delivering/initiating direct care work Experience in assessing, planning, implementing and evaluating care to meet service user needs. Knowledge of standard care plan and CPA framework Has a good working knowledge of psychological approaches in mental health Has an understanding of the relevant skills required and a commitment to developing skills in psychological thinking and interventions appropriate to skill level and in line with the service integrated care pathways Is willing to develop skills and knowledge in formulation and to work under supervision where required developing collaborative formulations with service users and families Will engage in the learning of students, professionals, carers and service users. Will support and supervise volunteers that work in the service Engages in partnerships with 3rd sector organisations, not just within the service but with all organisations that interface with the care of our service users including primary care (and other local agencies). To advise on mental health issues and support as necessary Values the importance of partnership working and the contributions this brings to clinical care/services Has experience of working with diverse populations and values the work associated to promoting diversity and social inclusion. To work flexibly over a shift pattern, mainly working day shifts Monday to Friday, with one weekend per month and occasional late shifts as the service requires, working arrangements might on the odd occasion vary to ensure that the service is adequately resourced. 2. Analysis, Problem Solving and Judgemental Skills The post holder has a responsibility to seek advice from the Band 7 staff or senior colleagues on clinical decisions that necessitates further/different health interventions. 3. Planning and Organisational Skills Prioritise Acute Inpatient actions for individuals on a daily basis Post holder will be involved in the risk assessment of individuals in conjunction with other members of the team, service users and their carers. They will ensure appropriate plans of action are in place to manage identified risks. Will cover and support team members during times of annual leave or unexpected leave of absences. To use professional judgement and exercise professional accountability in all aspects of clinical practice. 4. Physical Skills Health, Safety and Risk including Safeguarding You must at all times comply with the Leeds and York Partnership Foundation NHS Trusts Health & Safety Policies, in particular by following agreed safe working procedures and reporting incidents using the trusts risk incident reporting system. You are required to inform the line manager of any safety issues that could affect you or others in the work place. You are responsible for your own Health & Safety and must co-operate with the management at all times in achieving safer work processes and work places, particularly were it can impact on others. You will be trained in the correct use of any equipment provided to improve Safety and Health within the trust. You are required to use the equipment when necessary and as instructed; it is your responsibility to ensure the equipment is safe to use, prior to its use and must report any defects immediately to your manager. You are required to provide adequate supervision to ensure compliance with safe work practises. You will be expected to carry out risk assessments, identify hazards in your work place, and evaluate the level of risk associated with identified hazards and implement adequate controls to eliminate or reduce the level of risk. You will be expected to undertake safeguarding training (Children and Adults) appropriate to your role and adhere to policies and procedures relevant to the area you work in. Patient/Client Care In conjunction with the Ward Staff and under the supervision of the Band 7 staff or other senior colleagues to provide evidence based assessments and intervention (directly or indirectly) that improves the quality of life of service users requiring support to optimise their recovery. To be able to use the MOHOST as a screening tool and direct others in the use of the assessment To maintain a commitment to person centred care and to promote the use of the recovery model where appropriate engaging service users in all aspects of their care. Undertake direct clinical treatment interventions as required by the service user in differing settings. Collaborate with outside agencies to ensure care packages are in line with all agencies policies and procedures and current legislations. To promote social inclusion and diversity for clients by coordinating/facilitating and supporting access to a variety of community based opportunities including work, social groups, education, spiritual pursuits, leisure activities to ensure holistic wellbeing. To promote the physical health and wellbeing of clients by supporting assessment and interventions that facilitates access to relevant screening and information around therapeutic activity, medication, diet, nutrition, exercise, sensible drinking and smoking cessation To act at all times with courtesy towards Service users, relatives and other visitors to the unit and refer them to the senior team members as appropriate 6. Responsible for Human Resources You must take personal responsibility in agreement with your line manager for your own personal development by ensuring that continuous professional development remains a priority. You will undertake all mandatory training required for the role. Will provide supervision and appraisals for designated staff. Participate in clinical supervision as stated within Trust policy. Participate in PDP / Appraisal system In conjunction with line manager provide cover for the team caseload for holidays and sickness if required Assist in the mentorship and preceptorship of OTs. Will provide supervision and support to volunteers working within the service. To participate in the provision of an effective induction programme for new staff to achieve a clear awareness of roles and responsibilities, and operational policies and procedures.
