We are looking to recruit an Experienced Adult Social Worker to join our interdisciplinary team working with individuals with an HIV diagnosis. We are a team of Social Workers funded by the Sheffield Teaching Hospitals (STH) employed by the Local Authority on the NHS Foundation Trust's behalf. We work closely with our clinical colleagues at the Hallamshire Hospital.
Our focus is prevention, working in partnership with patients and families to help promote independence and enhance the quality of their lives. We support patients in the community and within the hospital environment. We are a regional service, and out-of-city visits and liaison with other Local Authorities are required.
Requirements within the agreed HIV Service Specification:
The post holder will work as a key member of the HIV Interdisciplinary Teams in the decision-making process with patients and their carers.
The post holder will provide assessment and support planning to meet needs and achieve better health outcomes, including:
* Prevention - early intervention to reduce delay for those needing formal support or directly delaying the need for social care intervention.
* Assessment and review in accordance with the Care Act 2014.
* Adults Safeguarding and Vulnerable Adults Risk Management.
* Attendance at multiple MDTs and close liaison with STH.
* Supporting people through the customer journey, i.e., named worker due to trust, confidentiality, and the long-term nature of work.
* Encouraging treatment and adherence.
* Support Sexual Health Services with contact tracing to reduce transmission and minimize public outbreaks.
* Provide psycho-social education.
* Work with families and children affected by HIV and HCV.
* Liaison with Jessops Hospital for pregnant women.
* Supporting marginalized customer groups who may not be eligible in accordance with the Care Act 2014.
* Flexible working, i.e., out of hours where required.
* Training for other professionals.
* Acting as a resource for social work and other services.
The Post Holder will have Specialist knowledge such as:
* Awareness of cultural assumptions, differing beliefs, and approaches to HIV.
* In-depth understanding of substance use, dependency, and associated behaviors.
* Understanding the physical and psychological impact of having blood-borne viruses.
* Knowledge of available treatment and side effects.
* Housing/rehousing, financial – benefits/hardship funds, support for clients to attend clinics.
* Case Management compliant with the Care Act 2014, Human Rights Assessment advice and support, asylum issues/immigration support.
* Adult Safeguarding and Vulnerable Adults Risk Management.
* Joint working with Children & Families Services regarding child protection concerns.
* Completion of MCA Assessments/Best Interest decisions when Social Care decisions are required.
