An exciting opportunity has arisen for an experienced cardiology/cardiothoracic nurse to join our Cardiac Health and Rehabilitation Programme in St Marys and Charing Cross Hospitals as part of the Imperial Healthcare Cardiovascular Health Services.
For a Bank Nurse within the Cardiac Rehabilitation service you will assess and manage out-patients and participate in the face-to-face Cardiac classes. You will act as a resource to other professionals on cardiovascular disease and associated treatments and issues. You will be taking part in ongoing audit and research activities to maintain quality care provision. You may also be required to provide occasional cover for our in-patient service at Hammersmith Hospital.
Imperial Healthcare NHS Trust, the UKs first academic health sciences centre, provides an internationally renowned tertiary cardiology and cardiothoracic clinical service and has a strong track record in both clinical and basic sciences research. For this challenging and fulfilling post we are seeking to recruit an enthusiastic individual with sound knowledge and understanding of the prevention and management of cardiovascular disease. The Cardiac Health and Rehabilitation Programme provides care to patients during the in-patient phase and to those who have been discharged and are attending the cardiac health and rehabilitation programme. It includes patients with cardiac disease, heart failure as well as those with ICDs.
Main duties of the job
To participate in the provision of information to patients and their carers regarding their disease or risk of disease, treatment and service available at all stages of their cardiovascular disease process either one to one or group sessions.
To participate in the teaching programme on cardiovascular disease risk factors for patients and relatives.
To support local patient support groups, or in their absence, facilitate the development of such groups, to facilitate user involvement when producing relevant work. To continue to support the local Heart Support Group. To maintain close links with Phase IV (community) exercise instructors.
Support the nursing team in the training and education needs of the multidisciplinary team and promote specific skills related to secondary prevention & rehabilitation of patients with cardiovascular disease. Work collaboratively with relevant educational staff/providers to deliver teaching programmes to meet these needs.
Updating own knowledge to promote excellence in clinical practice.
Job responsibilities
Support the cardiac rehab team day to day to ensure the programme runs effectively and efficiently.
* Provide in-patient services when required to identify and recruit patients and their families.
* Assess the risk factors and needs of patients with cardiovascular disease and plan an agreed package of care with the patient.
* In line with the cardiac rehab protocol undertake full nursing assessment, implementation and evaluation of patient, and families/carers. to provide specific expert nursing care, advice, support and counselling where necessary.
* To communicate complex information following assessments and analyse information which is sensitive to patients/carers and health professionals.
* Support the supervised exercise sessions of the cardiac rehab programme and deliver health education sessions as required.
* Aware of emergency procedure document to ensure safe running of classes.
* Participate in the co-ordination of patient care across the patient pathway, developing the pathway across the various settings to maximise patient satisfaction and outcomes.
* Attend Multidisciplinary Team Meeting(s), and advise its members and those of the wider health care community, to enable the provision of high quality patient care. This might include advice on physical care, symptom control, care planning, social and psychological care, communication, resources and equipment.
* Work with medical colleagues in ensuring that appropriate investigations are performed to complete the patients treatments and that families are fully informed and understand the purpose for these.
* Using motivational and counselling skills, to facilitate acceptance and ongoing compliance with treatment programmes where there are complex barriers to acceptance and difficulties in understanding e.g. psychosocial factors, lifestyle dynamics, understanding and conceptualisation, behavioural issues and varied perception due to cultural background.
* Act as patients advocate and counsel when informed discussion may lead to choices being made concerning treatment options or quality of life issues. Ensure that all relevant information is accurately documented in the patient health record.
* Maintain a support network and contact link for patients and/or families/carers during and following treatment.
* Manage own caseload and maintain patient records.
* To maintain good office procedures, accurate and up to date records and ensure the recording of statistical data in line with agreed procedures.
* Coordinating discharge planning for patients and their families including contacting external agencies.
* To be flexible with location and time of working to ensure accessibility of the programmes to all. This may include working on different site or in the evenings.

Qualifications
Essential
* First Level Registration
* Post registration qualification in cardiology or related area
* ENB 998 or equivalent
* ILS trained
* Minimum of 2 years recent experience in cardiology/cardiothoracic Experience in teaching, coaching or mentoring
* Experience of auditing and evaluating clinical nursing care
* Understanding of risk factor of CVD and the pathophysiology of CVD. Good understanding of Cardiac conditions and management of post-surgical care.
Desirable
* Delivering Health Education to individuals and/or groups
* Understanding of Cardiac Rehabilitation programmes and secondary prevention
* Knowledge of national policies & directives that impact on service delivery (BACPR, NICE)
* Understanding of the principles of health education and motivational interviewing
