Diagnose and manage patient presentations, using advanced clinical skills and judgement, supported by investigations such as blood tests, ultrasound and x-ray. Develop management plans, in conjunction with the patient and their wishes. Prescribe medication when indicated, in line with evidence-based practice and local/national guidelines, reviewing effectiveness and compliance when required. Refer, when appropriate, to secondary care or other services such as district nurses, physiotherapists, and audiology. Communicate and work collaboratively within the general practice team, to meet the needs of patients. Demonstate empathy and compassion when communicating advice to patients and relatives. Support the development of others within the practice. Maintain accurate, up-to-date healthcare records appropriate to the consultation. Encourage patients to take responsibility for managing their own care and treatment, where it is safe to do so, and provide health education. Take a holistic approach to patient care, bringing together their medical, social and psychological needs.
