Situated in the outskirts of Derby City, we have 12200 patients, provide services from one purpose built site and utilise the clinical system SystmOne.
You will be part of a team of 9 GPs supported by a highly skilled and motivated Nursing team consisting of 2 Practice Nurses, 1 HCA and a Nursing Associate. We have direct support from 3 Clinical Pharmacists, 1 Pharmacy Technician, First Contact Physiotherapist and a Social Prescriber, Care Co-Ordinator and Health and Wellbeing coaches.
We are a training practice for STs, medical students and student nurses. We have high achievement in QOF. We are actively engaged with our ICB with one of our partners being a Clinical Director within our PCN. We are passionate in assisting with future development of primary care within our locality. Main duties of the job
To be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.
Working as a member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.
The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, well women and well man clinics, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.
