An excitingopportunity has arisen to join our multi-disciplinary PCN Team due to thechanges in the ARRS Scheme, we are looking to expand our team by seeking toemploy a newly qualified General Practitioner to work alongside our practicesand PCN staff.
OurPCN staff currently includes Clinical Pharmacists, Social Prescribers, Health& Wellbeing Coach, Mental Health Practitioner, Paramedics, First ContactPhysiotherapist, GPAs, Pharmacist Technicians, PCN Manager/Digital &Transformation Lead and a Care Home Team.
Thesuccessful applicants will work alongside the PCN and practice teams to supporthealthcare professionals, patients, and carers.

Main duties of the job
To perform as anautonomous general practitioner working independently and with other healthcare professionals to assess, diagnose and treat the conditions of patientsattending within a primary care setting. Provide expert professional advice topatients, carers and colleagues and ensure the maintenance of clinicalexcellence. To be involved in the provision of education and training to GPRegistrars and other staff.
To be professionallyaccountable and responsible for all elements of clinical practice in accordancewith the GMC code of conduct ensuring the safety of patients and quality oftheir care is primary.
The post holder willneed to be capable of independent travel across the Preston and, South Ribble area.This role is a fixed term position for 6 months until the end of March 2025.

GP practices have been working together in our area for years and formed BridgedalePrimary Care Network (PCN).Bridegale Primary Care Network is made up of GP registered patient lists for Roslea Surgery, Riverside Medical Centre and the Ryan Medical Centre. We serve a community of around 30,000 people. We are small enough to provide the personal care valued by our people and large enough to have impact and economies of scale through better collaboration.
As a Primary Care Network we are working hard to deliver on the national NHS England network contract that commenced in July 2019 and continues to March 2024.

Clinicalresponsibilities
Inaccordance with the Practice/PCN rota, as agreed, the post-holder will make themselfavailable to undertake a variety of duties including surgery consultations,telephone consultations and queries, visiting patients at home and in Carehomes, checking and signing/authorising repeat prescriptions and dealing withqueries, paperwork, and correspondence in a timely fashion.
o Making professional, autonomousdecisions in relation to presenting problems, whether self-referred or referredfrom other health care workers within the organisation.
o Assessing the health care needs ofpatients with undifferentiated and undiagnosed problems
o Screening patients for disease riskfactors and early signs of illness
o In consultation with patients and inline with current practice disease management protocols, developing care plansfor health.
o Providing counselling and healtheducation
o Admitting or discharging patients toand from the caseload and referring to other care providers as appropriate
o Recording clear and contemporaneousconsultation notes to agreed standards.
o Collecting data for audit purposes
o Compiling and issuingcomputer-generated acute and repeat prescriptions
o Prescribing in accordance with thepractice prescribing formulary (or generically) whenever this is clinically appropriate.
o The post-holder will be required tovisit patients/care homes during surgery time in an emergency if clinically indicated.

Makeimproved use of the telephone and other technologies. Encourage patients torely on face-to-face consultation only where there is a clinical need to do so.
Communicateand reassure patients of the need to offer alternative care options whereappropriate and safe to do so.

Information Security
Under the provisions of the DataProtection act, it is the responsibility of each member of staff to ensure thatall personal data relating to patients and members of staff, whether held inmanual or electronic format, is always kept secure. Computer passwords must notbe shared either between systems or users. The Practices/PCN may monitor emailmessages, any files stored on the networks or in equipment and usage of Internet, and computer systems, irrespective of whether these relaters to PCN orpersonal use.
Access and usage of practices/PCNscomputers must be in accordance with the Practices/PCNs Policies. Safe havenprocedures are to be used for all electronic transfer of personal data. This isin order to protect the practices, patients and staff, and its reputation andto ensure that it complies with the laws and other guidelines.
