Responsibilities:
* Patient Care : Assess mental status, cognitive skills, and health issues of older adults.
* Medication Management : Organize and educate patients about their medications.
* Health Education : Provide information on personal safety, disease prevention, and lifestyle adjustments.
* Care Coordination : Link patients with community resources and manage care plans.
* Teamwork: Ability to work effectively within a multidisciplinary team
Skills and Competencies:
* Patience and Empathy : Essential for working with older adults.
* Communication : Strong listening and communication skills to balance patient and family needs.
* Clinical Knowledge : Understanding of geriatric health issues and treatments.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
