Job summary
This post provides an excellent opportunity for an enthusiastic, dynamic and motivated pharmacist with a clinical pharmacy background to join thePost-Acute Medicine team. The post holder will be responsible for leading and delivering a high quality, clinical service to a both post-acutemedical and stroke wards.
The successful candidate will have the opportunity to work across Post-AcuteMedicine which includes a broad range of specialities includingrespiratory, cystic fibrosis, stroke, gastroenterology, diabetes and elderly care. The pharmacists within Post-Acute Medicine provide extensivepharmaceutical care and medicines management services to patients within these areas to ensure a seamless integrated model of care.
Main duties of the job
The post holder will participate in the following services:
1. Act as a deputy for Principal Pharmacist when necessary.
2. Participate in the multidisciplinary team delivering and promoting high quality, cot-effective, patient-focused pharmaceutical care to patients.
3. Provide interface and discharge liaison, and support wards & units within the Care Group.
4. Consistently lead on the provision of clinical pharmacy services to the many sub-specialities within the Care Group and ensure a high level of clinical pharmacy input.
5. Act as a clinical role model, and supervise rotational staff at ward level.
6. Promote the safe drug use within the speciality.
7. Develop, implement and audit local guidelines.
8. Audit usage of medicines in the Care Group with the aim of promoting cost effective prescribing.
9. Assist in the financial reporting to the Care Group and drug budget management.
10. Ensure seamless pharmaceutical care on discharge, liaising with other pharmacists and healthcare professionals (primary and secondary) taking over care of the patient.
11. Work with the Principal Pharmacist, Post-Acute Medicine to extend the boundaries of professional practice within the Care Group.

12. MPharm (Masters Degree)
13. Registered Pharmacist (1 year competency assessed and examined professional registration with the General Pharmaceutical Council)
14. Postgraduate Certificate in Pharmacy Practice or equivalent qualification/experience
15. Working towards a Postgraduate Diploma in Pharmacy Practice or Clinical Pharmacy
Desirable
16. Registered Independent Prescriber or willingness to complete
17. Member of the RPS faculty
18. Postgraduate Diploma in Pharmacy Practice or equivalent qualification / experience
