An exciting job role has recently become available at the Bristol City Council. They are looking for a new member for their Adult Services team.
Key Responsibilities:
* Engage in a fast-paced, multi-disciplinary environment, working closely with health colleagues and community agencies.
* Demonstrate a thorough working knowledge of hospital pathways to support patient care.
* Conduct Care Act Assessments, Mental Capacity Assessments, and chair Best Interest Meetings .
* Carry out timely reviews as required, ensuring ongoing support for patients.
* Apply a Multi-Disciplinary Team (MDT) approach to ensure a safe and efficient discharge from the hospital that aligns with the service user’s needs.
Requirements:
* Proven experience in adult services within hospital settings.
* Excellent communication skills to effectively collaborate with a team and external partners.
* Proficient in completing assessments and reviews with precision and empathy
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
