We are a busy happy team who welcome new staff members and encourage staff development and learning. Due to a promotion, we are seeking an experienced nurse to join us in providing care and treatment to adults living in the area who have long term or complex health needs. The scope of your role will focus on supporting admission avoidance, early supported discharge, promotion of independence, and assisting in the effective management of patients with long term or complex conditions.
You will be working in a multi-disciplinary way alongside a range of primary care services including those from GPs, social, and voluntary sector. You will be responsible for managing your allocated work on a day-to-day basis, ensuring patients are prioritized according to clinical need and that assessments and care plans are recorded in line with the Trust's standards. Essential for this role is experience with line managing staff, the coordination of a shift, and supporting junior staff.
To support you within your role, we have an excellent learning and development department, and access to further develop your clinical skills and leadership development are available and encouraged.
Our service runs from 0700-1700, 7 days a week.
Access to a vehicle and possession of a full driving license is essential.
Main duties of the job
The primary role of the Gosport Community Team is to provide strong holistic leadership to the team and support the Integrated Senior Clinical Lead (band 7), including day-to-day leadership responsibilities, development and creative leadership, responsive leadership, enabling the team to achieve a 'well led' culture.
The community teams work in conjunction with a wide range of clinical colleagues and specifically, GPs, Practice Nurses, and Social Care professionals, will lead and facilitate a patient or client-focused, coordinated case management approach across primary and secondary care for people who are most vulnerable to, and at high risk of, repeat admissions to hospital.
The candidate will participate in and influence efforts across health and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management, and assure early intervention through the proactive provision of care in or as close to the patients' own home as possible.
The candidate will work across the caseload and the single point of access or equivalent, using their clinical skills to identify the needs of patients and the correct services to liaise with.
The candidate will provide expertise in their own discipline to the wider team.

Qualifications
* Registered General Nurse/Registered Mental Health Nurse/Practitioner or Registered Allied Health Professional
* Post registration qualification or University Degree
* Teaching and assessing qualification ENB 998 or equivalent/mentorship qualification - PETALS
* Ability to understand and interpret research findings/evidence-based care and apply to practice
* Good knowledge of health and safety and risk management
* Computer literate including emails and spreadsheets.
* Effective written and verbal communication skills
