We are seeking a dedicated and compassionate Community Nurse (Registered Nurse) to join our healthcare team in Melbourne. This role involves delivering high-quality nursing care to clients in their homes and the community, supporting individuals with diverse healthcare needs, and promoting independence and well-being.
As a Community Nurse, you will work autonomously and collaboratively within a multidisciplinary team to provide holistic care, health education, and support to clients, including elderly individuals, people with disabilities, and those recovering from illness or surgery.
Key Responsibilities
* Deliver person-centered nursing care in clients’ homes or community settings in accordance with Australian healthcare standards.
* Conduct comprehensive health assessments, develop individualized care plans, and provide ongoing monitoring and evaluation.
* Administer medications, wound care, catheter care, injections, and other clinical procedures as required.
* Educate clients and their families on health management, disease prevention, and lifestyle modifications.
* Support clients with chronic disease management, rehabilitation, and palliative care.
* Collaborate with general practitioners, allied health professionals, and other healthcare providers to ensure integrated care.
* Maintain accurate clinical records and documentation in compliance with healthcare regulations.
* Adhere to infection control protocols and workplace health and safety guidelines.
* Participate in case conferences, care planning meetings, and continuous professional development activities.
* Provide emotional support and advocacy for clients to enhance their well-being and independence.
Requirements
* Registered Nurse qualification (Bachelor’s Degree in Nursing or equivalent).
* Current AHPRA registration (or eligibility to obtain registration).
* Minimum of 2 years of relevant experience, preferably in community, aged care, home care, or primary health settings.
* Strong clinical assessment, wound care, and medication administration skills.
* Valid Australian driver’s license (or ability to obtain one) and access to a reliable vehicle.
* Excellent communication and interpersonal skills.
* Ability to work independently and within a team in a community setting.
* Understanding of Australian healthcare standards, infection control, and patient safety guidelines.
