Children & Young Person's Substance Use and Activities Worker
Adferiad Recovery delivers a flexible and coordinated response to the exceptional circumstances faced by people with co-occurring mental health and substance use conditions and related issues. Vulnerable people facing complex life challenges need consistent and seamless support to ensure they remain engaged with vital health and wellbeing services – and to prevent them becoming disenfranchised and isolated.
Hours: 37.5 hours per week, Permanent
Salary: £24,245.10 per annum
Based at: Neath Port Talbot
Job Purpose: **
In this role, you will carry out comprehensive substance use assessments, risk assessments, develop and review care plans and contribute to a holistic package of care for each individual. The post holder will also be responsible for organising and coordinating activities for the Children and Young Person service, and provide advice and information on substance use, one-to-one support, alcohol and drug awareness group sessions, and transitional support to adult services where appropriate.
The post holder will facilitate Brief Interventions, use Motivational Interviewing skills, and deliver Relapse Prevention sessions. The aim of the work carried out is to address substance use and associated behaviours, to improve relationships and enable young people to reach their full potential within a supportive environment.
