Are you a Nursing Associate looking for a supportive team and the opportunity to develop new skills? We have a new opportunity for a Band 4 Nursing Associate to join our Unplanned Care Team.
The aim of the service is to avoid unnecessary admissions into secondary care and provide a timely discharge from hospital and community bedded units for patients requiring urgent input in their usual home environment. The team provides care across three pathways delivered over 7-days a week 8am to 10pm - Rapid Response (RR), Early Intervention Vehicle (EIV) and Discharge to Assess (DTA). Unplanned Care also includes the Frailty Hospital at Home pathway. All pathways are delivered over the 4 localities in West Hertfordshire: Dacorum, Watford and Three Rivers, Hertsmere and St Albans and Harpenden, where you will be required to cross cover.
You will be supported to develop and achieve competencies to provide individualised care to patients who have long-term conditions and to complete various nursing assessments and following treatment regimes.
We are looking for forward thinking, passionate Nursing Associates (ANA’s will be considered if you are on the course) to support the management of patients within the South and West Hertfordshire localities when they have an unscheduled visit requirement.
The role requires you to be able to keep accurate clinical records and have good clerical/ administrative skills.

Candidates must hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
