An exciting opportunity has arisen for a dynamic and highly motivated individual to join a high performing Patient Safety Team at Newcastle Hospitals. We are seeking an inspirational leader with a drive to make a real difference to patient safety who has experience of working with a wide range of professionals at all levels within the healthcare setting.
In addition to contributing to key functions of the patient safety team, the post holder will take a lead role in mortality surveillance and monitoring, including analysing mortality data and presenting the findings at various forums within the Trust.
The successful candidate will be able to operate at a strategic level, play a key part in the continuing development of services and contribute to organizational learning.
Main duties of the job
This role works within the Patient Safety Team within the Clinical Governance and Risk Department.
The post holder will have a Trust-wide remit with responsibility for leading and facilitating developments in mortality surveillance, working closely with the Trust Medical Examiners. As we continue to transition to a new structure across the organisation, this post holder will also be responsible for supporting Clinical Boards with their quality and safety governance and will work closely with the Board Quality and Safety Leads as well as the Patient Safety Manager and Trust Patient Safety Specialists.
Job responsibilities
* To take a lead role on mortality surveillance and monitoring, including analysing mortality data produced locally and nationally and presenting findings to Trust groups, including Summary Hospital-level Mortality Indicator and VLAD data.
* To work closely with the Medical Examiners to ensure compliance with the expected standard of Level 1 and Level 2 mortality reviews.
* To work closely with the Clinical Boards in order to ensure Mortality and Morbidity meetings have an agreed agenda and that the quality of the reviews meets the standard as laid out in Trust Policy and the Royal College of Surgeons good practice guide.
* To support the Clinical Boards to review, analyse and interpret their mortality data and understand the significance of themes and trends.
* Support the Trust's patient safety and risk management function.
* To lead on the effective use of obtaining, interpreting and reporting on mortality data from data management systems.
* Using the risk management systems, support the Clinical Boards to use Datix for incident reporting and help them interpret this data in order to identify key patient safety and risk areas.
* To work closely with the Patient Safety Manager and Head of Patient Safety to ensure that appropriate risk management and patient safety related policies and processes are implemented effectively.
* To deputise for the Patient Safety Manager as required.
* Provision of a wide range of education for clinical and non-clinical staff, including incident investigator training, Trust Induction and other patient safety related education.
* To take a lead role in the analysis/incident investigation process to ensure lessons are learnt and shared appropriately, particularly in relation to Trust identified Patient Safety Incidents (PSIs) and safety themes, aligned to the Patient Safety Incident Response Framework (PSIRF), of the National Patient Safety Strategy.
* Responsible for the supervision and management of other staff, plays a key management role in the department.
* To contribute as a senior member of the Clinical Governance & Risk Department (CGARD) to broader developments as required.

Qualifications & Education
Essential
* Degree level qualification or equivalent highly developed specialist knowledge, underpinned by theory and experience.
* Post Graduate Qualification to Masters or equivalent experience.
Knowledge & Experience
Essential
* Knowledge and understanding of patient safety and clinical governance regulation and legislation, such as Duty of Candour.
* Experience of leading change/projects/ Quality Improvement.
* Detailed understanding of the concepts of Patient Safety and Risk Management.
* Experience of current generation IT systems.
* Experience of incident reporting systems and incident investigation.
Desirable
* Knowledge of Datix System.
* Knowledge of the Patient Safety Incident Response Framework.
* Knowledge of mortality surveillance and learning from death processes.
* Understanding of the regulatory requirements for NHS organisations in relation to patient safety.
* Experience of working in Newcastle upon Tyne Hospitals Foundation Trust.
