We care for patients, families and their friends, both before and after death.
Most people are cared for in their own home and our goal is to help people make the most of their lives, in ways that are important to THEM. This care may sometimes include coming into the Hospice as part of our wellbeing and reablement offer.
We are currently looking to recruit passionate, caring and experienced Healthcare Assistant Palliative Care Assistants to work within our growing Hospice In Your Home team.
As part of the Hospice in Your Home Team the post holder will work as a Palliative Care Assistant to support palliative and end of life care patients within their own home. This may include help with personal care, assistance with moving and handling and emotional support symptom management. The HIYH Team provides Hospice at Home and Hospice Night Support services over 24 hours, 7 days per week, including Bank Holidays and shifts are allocated on a roster basis. You will therefore be required to work in both
Main duties of the job
The post holder will work under the direct or indirect supervision of HIYH Registered Nurses and undertake tasks and duties as delegated by the HIYH RN Shift Lead. This includes supporting families and carers with helping patients to remain in their preferred place of care whilst working collaboratively with Community Nursing and Social Care Teams. The role requires travel within the geographical boundaries of Nottinghamshire Hospices referral criteria.
The post holder must have Health Care Assistant experience and possess a minimum of NVQ Level two or equivalent within Health and Social Care. Support will be given to the postholder to develop their career ambitions. You must possess a full UK driving license and have access to a vehicle.
