Job Description: Support Worker at UKAT

Shift Pattern
Mix of early shifts (07:40 am - 4:00 pm), late shifts (3:10 pm - 11:30 pm), and sleep shifts.

Main Responsibilities
* Provide advice, information, and support to clients, including referral to aftercare services.
* Develop and review recovery plans with clients.
* Ensure adherence to programme rules and support investigations of breaches.
* Support medication prompting, audits, and record-keeping.
* Assist clients with practical aspects of daily living and attend external appointments.
* Conduct intake, assessment, and discharge procedures.
* Participate in risk management planning and reviews.
* Maintain accurate client records and databases.
* Support the transfer of individuals between services.
Qualifications and Skills
* NVQ Level 2 in Health and Social Care or equivalent.
* Ability to work safely alone and influence professionally.
* Strong risk management and empathetic communication skills.
* Capacity to handle pressurized situations.
